Registered Nurse (RN) Case Manager – Hybrid Schedule (NO PREVIOUS CM EXPERIENCE REQUIRED)

We are seeking an experienced and compassionate RN Case Manager to join our team supporting members with complex medical and social needs. This hybrid role combines remote work with local field visits to ensure members receive high-quality, care.

No prior case management experience is required. This is an excellent opportunity for RNs who are looking to transition out of bedside care into a more autonomous, community-based role while continuing to make a meaningful impact.

In this role, you will provide comprehensive case management services, develop and monitor individualized care plans, and coordinate services across multiple care settings. You will work closely with physicians, interdisciplinary team members, and community providers while adhering to nursing standards and organizational policies.

Key Responsibilities

• Conduct initial and ongoing assessments of patients’ clinical, psychosocial, and environmental needs
• Develop and update individualized care plans in collaboration with the interdisciplinary team and in accordance with physician orders
• Complete MassHealth MDS assessments as required
• Provide education and support to members and their families
• Maintain communication with Commonwealth Care Alliance (CCA), Element Care staff, and community providers
• Manage a caseload of approximately 70–80 members once fully trained
• Participate in team knowledge-sharing and quality improvement initiatives
• Perform other duties as assigned

Training and Support

• Participate in a 2–4 week training period with experienced RN Case Managers
• Gradually transition into an independent caseload with ongoing support from the team

Required Qualifications

• Active Massachusetts RN license
• Minimum of 2 years of RN experience
• Ability to travel locally and conduct in-home assessments
• Strong clinical assessment and communication skills

Preferred Qualifications

• Experience in Home Health, VNA, PACE, or MDS assessments
• BSN degree
• Bilingual abilities
• CCM (Certified Case Manager) certification
